Job Application - Demographic Information

  • Ethnic Categories

    WHITE (not of Hispanic origin). All persons having origins in any of the peoples of Europe, North Africa, and the Middle East. BLACK (not of Hispanic origin). All persons having origins in any of the Black racial groups. ASIAN OR PACIFIC ISLANDER. All persons having origins in any of the original peoples of the Far East, Southeast Asia, or the Pacific Islands. This area includes, for example China, Japan, Korea, the Philippine Islands, and Samoas. Also persons from the Indian subcontinent, including peoples with national origins for Bangladesh, Bhutan, India, Nepal, Pakistan, Sukkim and Sri Lanka. AMERICAN INDIAN OR ALASKAN NATIVE. All persons having origins in any of the original peoples of North America. HISPANIC.   • Veteran Status

    VETERAN – A person who 1) served on active duty for a period of more than 180 days, 2) who received other than dishonorable discharge, 3) who does not fall into any of the other categories outlined below. DISABLED VETERAN – A person who has 30 percent or more disability and is entitled to disability compensation by the Veteran’s Administration who was released from the military service (active duty) for a disability incurred or aggravated in the line of duty. VIETNAM ERA VETERAN - A person who served on active duty for more than 180 days (any part of which was performed during the period from August 5, 1964 through May 7, 1975) and who was discharged or released from the military service with other than a dishonorable discharge. SPECIAL DISABLED VETERAN, is a veteran who (1) is entitled to compensation (or who but for the receipt of military retired pay would be entitled to compensation) under laws administered by the Veterans Administration for a disability (a) rated at 30 percent or more, or (b) rated at 10 or 20 percent in the case of a veteran who has been determined under 38 U.S.C. § 1506 to have a serious employment handicap; or (2) a person who was discharged or released from active duty because of a service-connected disability. “ARMED FORCES SERVICE MEDAL VETERAN”- means a veteran who, while serving on active duty in the U.S. military ground, naval, or air service, participated in a United States military operation for which an Armed Forces service medal was awarded pursuant to Executive Order 12985 (61 Fed. Reg. 1209).
